As of January 1, 2014, the New York State Workers’ Compensation Board (WCB) put in place new filing requirements in response to the change in method of WCB assessment calculation:

All municipal self-insurers, including all members of PERMA, are required to report total payroll to the WCB quarterly.

Starting with the first quarter of 2014 (ending 3/31/2014) PERMA’s Auditing Department will need to collect, from every member, a copy of the NYS-45 form. The form will need to show ONLY the total payroll for the quarter (not the detail).

FIRST QUARTER PROCEDURE

  1. PERMA must receive the NYS-45 form showing the total payroll from you by 4/15/2014.

  3. PERMA must file with the WCB on behalf of the entire PERMA pool by 4/30/14. The WCB will levy fines for late reporting.

*PERMA will do all we can to make this process as easy as possible*

Please note that this is a separate filing requirement from the request for your PERMA annual payroll audit. But it is also a temporary requirement, as the WCB plans to establish a payment process for assessments directly with employers.
