As of January 1, 2014, the New York State Workers’ Compensation Board (WCB) put in place new filing requirements in response to the change in method of WCB assessment calculation:
All municipal self-insurers, including all members of PERMA, are required to report total payroll to the WCB quarterly.
Starting with the first quarter of 2014 (ending 3/31/2014) PERMA’s Auditing Department will need to collect, from every member, a copy of the NYS-45 form. The form will need to show ONLY the total payroll for the quarter (not the detail).
FIRST QUARTER PROCEDURE
- PERMA must receive the NYS-45 form showing the total payroll from you by 4/15/2014.
- Please send the form in to PERMA (Attention: Jennifer Schacht):
- PERMA must file with the WCB on behalf of the entire PERMA pool by 4/30/14. The WCB will levy fines for late reporting.
*PERMA will do all we can to make this process as easy as possible*
Please note that this is a separate filing requirement from the request for your PERMA annual payroll audit. But it is also a temporary requirement, as the WCB plans to establish a payment process for assessments directly with employers.
More information will be forwarded as soon as it becomes available and an informational webinar will be posted to the PERMA website in mid-February.
